Collaborative Health Partners Improves Quality Care For Medicare Patients in Southern Virginia

September 30, 2018 - Lynchburg, VA - One of Southern Virginia’s top clinically-integrated physician networks, Collaborative Health Partners of Virginia (CHP), today announced its success as a participant with one of the nation’s highest-performing accountable care organizations (ACOs), Privia Quality Network (PQN).

CHP’s primary care groups work to provide quality care at reduced costs for Medicare beneficiaries in Southern Virginia, which contributes to the successes of the entire ACO. As a group, PQN achieved shared savings of $38 million and a 95% quality score for its 2017 performance in the Medicare Shared Savings Program (MSSP). These achievements come from utilizing data analytics, care coordination, and care teams to deliver exceptional care.
