CHP Seeks To Expand Direct Care Services

In an effort to continue building on CHP’s commitment to value, CHP is expanding its services for Direct Primary Care, as well as Direct Care, to include a variety of medical services for both employers and general consumers.

CHP’s Direct Primary Care model was designed to improve access to preventive care, sick care, physician-directed wellness resources, and physician-directed disease management services while reducing the costs of healthcare for employees and employers.

As CHP looks to expand its Direct Care services, CHP is seeking additional partners in the healthcare community, to include health systems, hospitals, specialists, insurance companies, brokers, and others. Specifically, partners who are interested and committed to creating a nationally recognized model of care that improves quality and lowers the cost of care across the health spectrum.

Early partners who have already committed to these efforts are CVFP Medical Group, Walk-In Care Immediate Cares, Rehab Associates of Central Virginia, Liberty Mountain Medical Group, and Privia Medical Group. CHP hopes more local partners will soon join this effort!

At Collaborative Health Partners (CHP) we believe in creating value for our healthcare consumers. This requires our physicians and health systems to provide high-quality healthcare services to patients in the most cost-effective manner.

Current CHP partners have already proven the ability to improve quality and reduce costs within programs such as Medicare, Anthem, United Healthcare, and Humana.
